Our diabetes outpatient services help patients with diabetes develop and practice self-management skills that will improve their health status and quality of life and reduce the risk of diabetes-related complications.
The 12-month Program
Assesses patient's readiness to make critical lifestyle changes and supports them in that process through:
- A core education series that provides basic skills and information recommended by the American Diabetes Association
- Advanced education modules that focus on specific needs, such as weight management, insulin administration, oral medications and exercise
- Support groups that offer additional diabetes management education, as well as the opportunity to share experiences with other individuals who have diabetes
- Specialized education, including insulin pump use and the management of diabetes during pregnancy
- Periodic assessments of each patient's progress in reducing Hemoglobin A1c levels and improving self-management skills
- Coverage by most major medical insurance companies, including Medicare
