MGH Community Health is a JCAHO-accredited, full-service home health agency that can provide care in your residence. Both Home Health and Personal Care services can make your transition from the hospital to your home a safe and comforting one.
Home Health offers nursing services, including Infusion and mental health services, physical therapy, occupational therapy, speech language therapy, home care aides and social work services. Our infusion therapy assists the patient and family in becoming more independent in caring for their IV line and medication infusion.

Personal Care provides home care aides services, which can assist with bathing, dressing, grooming, ambulation, meal preparation, grocery shopping and light housekeeping duties. Personal Care can also provide skilled nursing services and a Personal Emergency Response System. Health Watch is activated by a transmitter worn around the wrist or neck and will connect you to Response Personnel, through your phone line, 24 hours a day, 7 days a week.
